What to bring to your appointment
- Referral letter from your General Practitioner (valid for 12 months, unless stated) or Specialist
- Medicare card
- Pension / DVA card
- List of current medications (including dosages)
- Relevant test results
- Completed Patient Registration Form if you are a new patient (can be also be submitted via fax or email)
- Completed Patient Detail Update Form if necessary.
Please arrive at least 10 minutes prior to your scheduled appointment time.
How should I prepare for my tests?
We recommend that you wear a two piece outfit to allow for the removal of the top half of your clothing for any of the tests. When you make an appointment, we will advise you if there are any specific requirements to prepare for the test you are having.
What do I do if I cannot attend a scheduled appointment?
If you need to reschedule an appointment, please contact us 03 9347 6584 or 0405 869 177
as soon as possible and we will reschedule your appointment to another time more suitable. This will allow other patients the opportunity to use these times.
Is there any parking ?
When visiting our main clinic in Carlton, please follow the signs outside the building that directs you to the car park via Cardigan Terrace. You are allowed to park in the bays reserved exclusively for our patients labelled as “Hotel Guest Parking”.
How to get to the clinic using public transport?
There are many trams that can get you right to our doorstep. For other public transport options, visit www.ptv.vic.gov.au.
